looks sort of like a fungus to me, or like its been attacked.
1) what tankmates?
2) what tank parameters?
3) when was the last pwc?
4) has is been bullied or chased by a specific tankmate before?

I'd suggest you check out this link and see if any of the symptoms match this diagnosis. Columnaris - Flexibacter columnaris My treatment of choice for this condition is Furan 2 as it battles both gram + and gram - bacterias
