My male jack dempsey started twitch and shake his head recently. I only notice it at night and early morning. He would swim around rubbing and twitching against the glass, rocks, and decor. The only other tank mate is another jack dempsey, which I believe is a girl, and she would go up to him in what looks like a pecking motion on his side most of the time he does it. I’m not sure if it’s a mating thing since I’m not really certain of the gender. I’ve had the small JD for over month before his actions started happening. They live in a 65 gallon, weekly cleaning, and everything else set for their needs. (Pics a little older)

Look very closely at the body, gills, vent, mouth and look for fish lice, small dots, moving specs, and or worms - anything which shouldn't be there.

Might very well be Trichodina.....a skin parasite. Usually doesn't display anything on the skin until a heavy infestation then you'll see white spots.
Methylene Blue, malachite green, Acriflavin are meds for this.
Sounds also like ich but more than likely you'd see the salt like dots on the skin. Velvet sometimes brings out similar behavior.
Keep us updated and check him out closely as Autumn suggested.

I don’t notice anything wrong with him physical. I’ll be cleaning the tank tonight, but his behavior been consistent even with the cleanings.

If I give him the medication you suggested will it harm him even if he doesn’t have it? I posted pictures of him in reply to Autumn. Also, can you tell me the gender of the Jack Dempsey below? Sorry for all the questions.

You could try Kordon's Methylene Blue in a quarantine tank with no carbon in your filters. Or monitor him for awhile longer and see if he starts to get better or worse.

I cant see a full body pic but looks male to me.
