What do you put over your powerhead intake?

I've been reading several posts in the forums where people have lost fish due to the fish getting sucked up into the powerhead(s). Do any powerheads come with guards to prevent this? If not, any tried and true methods for covering the intake on the powerheads?

Bearfan: Were these foam guards specfically designed for the Maxi-Jets, or did you buy some foam and cut it to fit?
