adf's dying!! plz help!!

Had 3 adfs in a 15 gal tank, water parameters everyhing 0 Ph is fine, they eat fine no cloudy eyes or anything now in the past like 4 days 2 of them have suddlenly died I only have one now. I got them from Petsmart one month ago exactly. have been doing weekly 60% pcw. What is going on?!?! Please help me I dont want my last one to die!

Hi I also got some AFD'S and they dont really like water changes I only do 25% and they seem to be happy with it also it is possible u are stressing them out u could buy some amphibian stress Zyme

Also are there lots if hiding spaces they really like that and will sit there all day
