How to clean without starving goby?

Do I need to vacuum substrate like sand and medium grade if I have sand sifting stars and gobies?

If I do, then how do I get the poop out without killing the fauna?
 
If you have a cleanup crew, then all you should ever have to do is spot vacuum the poop. If there is one place where it sits, try adjusting a ph so that the flow keeps it from collecting in a dead zone.
 
Does the cleanup crew eat poop? or does the fish waste turn into something they can eat?
 
Your best bet with the substrate will be a good amount of nassarius snails. Your puffer is both a messy and large eater and produces alot of waste. Nassarius is a good sand dwelling snail that mainly feed on detritus and other leftover wastes.
