How often should I replace my filter floss and carbon?

It maybe to often, but I do a 25% water and filter change every two to three weeks

Mechanical filtration should be changed out weekly during water changes. This way you are removing the 'chunks' that will degrade into your nitrates quickly. With that said, some do it even more often than that, changing filter socks out every couple of days.
