Water Testing Checklist

How do you all keep track of your water testing, has anyone made a chart to follow.? I was getting ready to make somekind of chart in excel, just wondering if anyone has something made up.?

I did a quick and dirty table in Excel to follow my tank progress... and then set up graphs to easily see the changes in levels... yes, I am a science dork!
I tried keeping everything in a book and was graph it out by hand, and then I realized that it was much easier to just enter it into Excel.
