do I turn OFF/UNPLUG the heater when I do a water change?

If you are JUST doing a water change, I think it is safe to leave it plugged in - but if you're planning on having your hands in the tank you may want to be safe and unplug it.
 
If I'm doing a 50% water change, I unplug it, if less, I usually leave it plugged in, as long as (as mentioned above) I'm not doing anything else in the tank.
 
Depends upon how much water I'm changing. If it's going to bring the level down to the heater, then I turn it off.
