I'm keeping 4 guppies in a 5 gallons tank right now. I'm wondering if it's possible to give them tank mates (preferred some kind of medium size fish). If not then would it be fine to drop some shrimp or snail in there?

Shrimp or a snail would be fine. 5 gallons is too small to have any medium sized fish in it.

Maybe this will help a bit.. quick google search:

“Here is a list of some of the most common guppy tank mates:

Livebearers (mollies, platies, swords)
Gourami Fish
Cory catfish
Rasboras
Most Tetra Fish
Small Species Plecos
Cherry Shrimp
Oto Catfish”

If the tank was bigger yes, but it's only 5 gallons so only the shrimp or snails would work.
