I have had this for about a month now. Its receded around the edges and all polyps are hard to the touch. I've never seen feeders even placing food directly on it. Still has color but, is it dead?

The recession is what you should be worried about. Sometimes coral simply don't inflate or extend feeder tentacles. Have you checked your alk levels? Could be playing a part in this issue. As could irritation from the sand.
 
I've had it on sand and rocks, algea got ahold of exposed bone so brushed it off and planted it to stop the algea.

Just tested my water.
Ammonia 0
Trites 0
Trates 5 ppm (I can't get lower)
Calcium 460
Alk 10
 
I'm running a skimmer, phosguard and chemipure elite.
 
I'm trying..doing 8 gallon water changes every Friday. A 15 about once a month. Everything else seems fine. Hammer and torch stay open and swaying. Frogspawn has recovered from dropping all its small polyps when I introduced it to my tank. Shrooms, zoo's and candy cane all open.
