Have you introduced anything new to the tank? maybe flatworms or something like that?

I'm not sure if this is related to your problem or not, but calcium and pH could be higher (e.g., calcium 400-450ppm, pH 8.2-8.4). What's are your alkalinity, phospahate and salinity levels?
