Feather duster out of its tube

Large feather duster was doing fine for 2 months in new tank. Did 10% WC on Monday. Feather duster left it's tube and went into a hole. We could see it in there yesterday but it was closed up. Today we checked again ad there is no sight of it.
 
Stress will make them loose their heads but most usually starve to death from what I have read and seen. I had 2 and they were both death within 6 months. I was target feeding but I am sure they starved.
 
These are the water parameters
Ph= 8.1
amo= 0
nitrite=0
nitrate=3ppm
Hydrometer= 1.022
temp= 75.7

Water clarity is very good
Corals seem to be happy
 
Its hard to tell exactly why. Two weeks ago mine lost its crown. I've had it in the tank for I don't know 8 - 9 months. I have a CoCo Worm that is doing just fine in the same tank. My feather duster never left its tube though, it regrew a crown, so far its a lot smaller, but at least its growing.
