Hammerhead Coral Help Needed!!!!

I have a hammerhead coral that is in my 155 gal tank, good position, good flow, etc. I have other pieces of the same that are doing just fine. This one however has been dying and now looks like its melting. Any advice!? Please help!!

It`s called a hammer coral. Do you see a brown jelly around the base of the tenticles. There`s a thing called brown jelly disease that will eat away at LPS corals.
