Yes Nitrites in any amount are lethal. Most likely your tank was not finished cycling. Moving this to the sick fish forum. Sorry for your loss.

krustykrab99 hears surf in the shell
it's always safe to assume that the fish is infected. along with good water condtioning treat with broad spectrum antibiotics. take out carbon and raise the temp a few degrees. anytime i see one acting different i immediately add antibiotics and I am a big fan of malachite blue for parasites.

I beleive you mean "Methylene Blue" whihc should never be used in a display tank. Placing a fish in QT for a month before adding it to the display tank is the generally accepted procedure for SW.
