Lost Picasso clownish

He was in a 40 breeder with another Picasso added same time. My parameters are 0. 0. 10. There is two fire fish and a yellowtail damsel and a few soft coral in. He was in the tank almost two weeks eating good micro pellets with garlic in morning and mysis shrimp at night. Not breathing hard or anything. Than found him stuck to Power head
This morning any ideas the only thing I did was water change and added a sea star serpent.
