My fish's head is eroding......help!

I have a yellow fang blennie and its head is slowly eroding. It started out as a white blotch at the top of it's head and now it looks like its creeping all over the top of his head. He disappeared for a couple days so we thought he died and became crab food but he suddenly reappeared today looking worse than ever. He has been very lethargic. We did a 50% water change about 4 days ago and a 5% change everyday since because we are batteling some major red slime algae. Haven't done the water specs today. Please if you know what this is and can help let me know.

hole-in-head disease? Lateral line erosion? Maybe one of these?
