I think I found your issue, I just happened to be surfing the interwebs and I came across a picture that looked very similar to what yours did.

Starfish Spawning.

I just saw all of my replies just now. Sad news, the starfish died. We were told that all the starfish in the tank we purchased him from died as well. Our perimeters were fine we even tried spot feeding. I don't think it was our fault.
