Sick BTA???

So my BTA every now and again it'll close up and get real small, almost looking like a little piece of leather. The thing is it only does it 2 or 3 times a week and then comes out bigger, fuller and more "active" with my clownfish afterwards. Now I know you'll want some info so here is what i can think of.

Water parameters are ok (see photo)

30 gallon tank, 24" Oydessa 4 bulb T5 2 whites, 2 blues, feed it every Wed & Sun night at 5pm, adequate flow

I'd say he is just expelling waste, or pretty much just pooping. lol Your nitrates are a bit high though, I'd aim for 10 or less with a nem in the tank.
 
Your high trates could be killing it. Also, that's really not a lot of light for a nem. Do a 25-30% pwc. Retest after 24 hrs.
