Sick star polyp

We have had our star polyp for almost a year now. It's been doing great after almost loosing it to an ich outbreak in September. Recently it's not coming out as much and it just doesn't look good. A few weeks ago I picked it up because it looked like it had moved slightly. Almost the whole under base of it is gone. Not sure if something was eating at it.
The most recent thing we added to the tank were Caribbean peppermint shrimp to rid of the aptasia. They did their job great getting rid of the aptasia but could they have eaten the star polyp? Prior to the shrimp we also had a lot of bristle worms which are now gone too. Could the worms have been eating it?
