What is going with my Anemone

My anemone started to (what looked like) turning himself inside out. but it still looked health. Now over the last day, it's been looking more and more like the pictures. What is it doing?

My guess is it is on it's way out. Poor lighting and water conditions are usually the cause.

How long has your tank been setup? What are your water parameters? What kind of lighting do you have on the tank?
