White patch comes and goes

Any ideas what this could be? He's active and feeds every time, looks health other than the white patch on his back, all other fish are fine. My salinity is a bit hi but not life threatening and every thing else seems to be heathy,

I had one that had that on its head. From what I understand is the females can begin to turn a gray color with dark bands around the tail. "She" decided to go for a swim and and got Into the overflow. My new one doesn't have the white spot but has lighter bands beginning to show around the tail.
 
So ten days on and things have got really bad for this little guy, he has a huge hole in his body, I can see its spine and insides but he won't die? I found my blood shrimp eating it alive today, pretty disturbing really!! Any one know what's going on? None of my other fish have this problem and look healthy

I can't catch it to QT it, tried a bottle trap but it feeds only when food floats by so baiting the trap up just catches all my other fish, I thing it's to far gone to save him but I would like to know what the problem is? Any one know, could it be a skin condition or maybe a parasite?
