Yellow tang

Now my yellow tang has some reddish spots on his head, lethargic, frayed fins, looks like he is dying. He is breathing fast and looks skinny. Any thoughts and what I should do?

What are water parameters, tank size, other occupants and what have you been feeding?

Tang is dead. And now I am concerned about my anemone. There is some white discharge coming off of his body. Kinda looks like cob webs. I haven't checked my levels in the last 2 days. I will ASAP.

He is shrinking up too.
