angel in pain

i recently bought a beautiful angel (about 3 inches)...he has been in the 5g qt tank for about 4 days...and i don't think he is comfortable at all...
symptoms -
his/her fins, specially the dorsal, the anal and the tail fins are usually clamped...there is a small tear in the middle of his tail fin that i had not noticed while buying...
he stays in the same position and rocks back and forth (shimmy?)...
sometimes he tries to jerk around as if trying to shake something off...(parasites?)

i have tried adding melafix and half a tsp salt...i do daily 20% pwc...

Does he/she have any visible symptoms other than the clamped fins? Check this to see if it helps:

Is your tank heated? How long has the QT been up and running? Did you rinse/clean the tank before you set it up? If so, did you use any chemicals?

What are the water parameters, including temp and pH? Are the eyes clear?

thanks guys ....
the qt has been running for 4 days...i set it up the same time i brought the angel...
washed the qt with bleach before putting it away...that was about a fortnight ago...just rinsed the qt and reused it...
i'm currently outta test strips... ...but the regular 20% change ought to keep things in check...

the eyes are clear...and good news is he's eating...

It could be as simple as stress. Is there a place for it to hide in the QT tank?

umm...not really...there's a big sponge filter...i'll put some plastic plants in and see how it goes...

well, not very well...i didn't see any improvement for the past couple of days...even after adding some cover...still clamped fins...
so i started a dose of jungles parasite clear tabs yesterday...
so far he's been maintaining a healthy appetite...
i'll let u know after 2 days...
