Bettas fins are ripped and torn

Hi,
I was just wondering if any one could help me with my Betta?
He's normally been very healthy but a few days ago I noticed his fins were ripped up and torn. He is still eating like his normal pig self but he seems lethargic. The shrimp in the aquarium looks fine but the Otto I have is darting around and acting skittish. I added some extra stress coat plus and did a forty percent water change. I could only get one pic below. Can anyone suggest anything else?

I'd get the store to test your water or grab a test kit yourself. Just a first check in case for ammonia and ph.

My fish's tail got torn up, I got something called like Melafix from store that helped her to heal up, it also helps with bacteria so they don't get infection
