Green Puff scratching against filter sponge

Hi folks,

I noticed a strange behaviour on one of my puff. He started scratching one side of his head on the sponge filter. Sometimes he does that on the wall, but more often on the sponge. Does this mean the beginning of ich? Shall I start treating the tank now?

Advice appreciated. Thx!
