Help! Betta sick!!

My betta is 1.5 years.

She just moved into a 5 gal fluval chi. She was in a 2.5. I started her new tank with her in it a week ago. She ate well. Swam well but this morning I find her on the bottom with a white film on her face. Her gill movements look labored. I did a 25% water change. Prior to change her amonia, nitrites, and nitrate were all 0.0. Her water temp is 74 F.

Advice please.
 
Began a treatment of Neoplex to fight a fungus. (Aquarium store advice) The fish swims quickly to gulp air then falls and lands hard on the bottom. While lying on the bottom gills are moving. Swimming seems to without control.

Any advice?
 
Keep the water clean and go half dosage for a longer period of time on any medications. Some fish, like bettas, can be extremely sensitive to medications so I found by doing half the daily dose for an expanded amount of time (which would end be the same dosage, just not all at once) helps.
