Help : Tetra gone downhill

Hi , my black skirt tetra has been sick for a while now with lymphocytsis (sp?) , he was getting better , but now he just looks really bad.ImageUploadedByAquarium Advice1395686837.869016.jpg
His body is all swollen , he had string white poop yesterday , half his dorsal fin is missing and he hides a lot. The phot was taken around feeding time , when he's out and about .
It's a Fluval Roma 125 tank , with a Fluval 305 external filter. Schools of black skirt tetras , penguin tetras , cardinal tetras and a BN pleco. Feeding Hikari micro pellets. Parameters 0.25,0,0 pH 6. What should I do ? I have 2 other tanks - a plant-only 5 gallon (no heater) and a plant-only 20 gallon walstad (heated , but with high ammonia as I'm doing a fishless cycle) . Would a transfer be worthwhile ?
 
Please help me :( He has red streaks on his body now . It's upsetting to see this and not know what to do ..
