Hole in goldfish gill plate??

One of my goldfish randomly got a hole in his gill plate. He has not been seen flashing and he seems to act like normal, very energetic. I do have detritus worms and have been doing weekly water change(s) for the past month. I've had my 10g fish tank for around 2 months. I don't know what could have caused it to form as it appeared within one night? Any advice?? I'll add a photo
