Sorry about the bad pics. Tank is bright and he moves around a lot.

Parameters are normal. Spots just popped up after I changed the substrate, but all other fish are fine.

I can't tell from the picture. Too blurry. Do the spots look like grains of salt sprinkled on the fish or fuzzy, irregular or slimly and white (or brownish)?

It's not ich then. Does the flakey one look like a scale coming off? Does the bump near the tail have anything protruding or a string hanging off it?
If don't have any salt sensitive fish you can add some salt to the tank to see if it clears up. 1 flat tablespoon per 5 gallons if water. Add it slowly over a day or so.
