Mouth problem?

Hey guys! Sadly noticed my Zebra d fish not acting itself today, I looked at him and it looks like his is having a bit of a mouth problem?
He keeps his mouth wide open now and he is not active at all, looked in his mouth and theres nothing in it but it looks kind of like his mouth is slowly disappearing and his skin is hanging and flapping well he breaths, other Zebras look perfectly fine but this guy...
Any ideas? Feeling bad for him.. Lol

Got worse over a little while, he started to be able to half close his mouth and ripped a little hole under his mouth now..and freaked out and made it worse .. What should I do now....
