oto looks beat-up - ideas on what may have happened?

When I checked on my tank this morning, I noticed that one of my oto cats was resting on leaf, but he had what looked like a line of pus down his back and another around his med-section, a bit behind his eyes. There was also a red spot that looked like blood on his side, roughly the same area as the anal fin. Here are the best pictures I could get of him (which I know don't show much but he slipped behind the heater). There are two otos in both pictures, the one farthest away and on the right is the one I'm concerned about.

Ammonia & nitrite are both 0, nitrate is a bit below 10, and water temp is about 78.6. Other inhabitants are Bolivian Rams, black skirt tetras, misc cory cats, dwarf gourami, so no body to aggressive.

Any idea whether the little guy could be sick or if somebody got to him?
