Problem with moving fish.

I'm trying to move a fish to a different tank and for some reason everytime I move it, it looks extremely bad. Really dark and clamped fins.
Water parameters are the exact same with 1 hour acclimation and this is the second time I've tried to move it.
Could someone help me out with why?

Might be just stress still but would the water be the same in terms of water age and TDS roughly? My 150gal can get close to the 5 gal in ph and nitrates but they are way different in gh / solids.

Those are signs of stress. Some fish acclimate faster than others.
