Unhealthy clown loach

I have had a clown loach for over 3 years, always fat and healthy. Over the past 2 months he has lost some weight and isn't moving around like he used to. But last 2 weeks it has been much worse. His head is sinking in. He is looking very bad. All my water params have been good and all of the other fish are healthy. I feed once a day with pellets, flakes, and granules and blood worms once a weeks. Any suggestions for how to help, or what it even is.
