Black and white worm

Hello Everyone,
I was hopeing that someone could help me identify this worm like creature. I have had my tank running for over one year now and this is the first time that I have ever seen this thing.

What is looks like is a black worm with white spots on it. It seems to move like the childs toy when you fill a balloon with water and squeeze it rolls on too itself. Lastly the mouth has tenticles when it goes too eat.

Here are the pictures hope they help.
