Emmmm bubble tips what I thought but I've never seen one look so empty, they normally look full. I bought a small tank which I'm going to use as a quarantine tank and this came with it.
 
Looks like a sebae anemone (Heteractis crispa) to me, agree its bleached out.

Edit: after blowing the image up on my phone (shouldn't try id's on a phone) I would lean towards a bta, but sebae is still a possibility.
 
They can bleach for many different reason, poor environment and stress are probably top of the list in the aquarium.
 
They bleach when they expel their zooxanthellae and can slowly die out because of it.
 
Make sure u try feeding it small meaty foods (mysis, brine, etc) at least once a day. No zooxanthelle means no food for the little guy, so u need to supplement its food intake. It may not take food for awhile but u need to try, their chance's of survival go way up
