I agree with everything except #2, we've been doing it for years, but we do have very good local water quality. Used tank water is definitely the safer option.

Yeah # 2 is a little off, I think. If anything, overfeeding and the resulting issues are far more prevalent IMO.

Thats a good list. I am surprized that underfeeding was listed. I have certainly seen underfeeding issues from some people who do not take good care of their tanks, but I would have guessed over-feeding to be a bigger issue. Still, just saying "feeding incorrect amounts" is how I would have worded that.
