Amazon sword not happy

Can anyone identify what is happening to my amazon sword? Not sure what I need to do to correct it.

Do you fertilize it with root tabs?

Yes there is new growth that is bright green and looks healthy. I've since trimmed out the unhealthy stems.

No root tabs, haven't tried those yet. I do flourish twice weekly and daily co2 dose, and the substrate is an organic soil. I originally thought the real soil would provide all the nutrients they needed.

On amazon swords, emersed form leaves have thin stems that lead up to the leaf. Submersed form leaves are broad all the way down to the base. The emersed stems usually die off as the plant gets settled in under water.
