Are your swords still growing new leaves? I read that some plants acclimate(not all the time) to the new water parameters and old leaves rots. And that as long as new leaves are forming your plant is ok. You just need to trim the old leaves. That's what I read.
I bought some ludwigia ovalis and all the old leaves rotted and fell off but new leaves were forming. I cut the new growth and planted it and it's been growing like crazy eversince. It's a different case with your swords though...
