Mystery Plant and Possible Deficiency?

I've had three of these plants in my tank since late October early November and since then, I have had 0 idea as to what in the world they are. Some of the leaves are a bright green, while others show signs of browning and wavy edges, sometimes curling towards the ends. I'd love it if someone could ID these plants for me, and maybe identify a possible deficiency. :)

Looks to be a crypt...place a root tab near the base of the plant,push below the substrate 1"+...crypt's are heavy root feeders&with the addition of root tabs will grow well..

Getcha sleeves wet fish tank people!
 
I use root tabs under most all of my plants, but the plant pictured is the only one of it's kind that has a root tab directly under it. When I put the first tab under it, it perked up for a couple weeks but now it doesn't seem to be working as well anymore. How often should tabs be replaced?
