I have had this plant for a while now in my tank and it lived, barely. But now it should grow better due to the better lighting. Anyway, I'd like to know what kind it is. Here's some pics:

Sorry about the quality. I didn't know how bad they looked until they were already on the computer. If you need better ones just ask. I edited them a little to try to make it more visible.

Again it's all the same plant. Also, I saw this on one of the leaves on my java fern:

What do you think could be the problem? It's only on one of the leaves, so, should I be worried? Thanks.
 
Looked at some pics and it does look a lot like the hornwort. Hopefully with the better lighting it will fill out some more. Do you think I should try placing it in the gravel? Although, it's kind of hard to make em stay. lol

Yeah, I always used some weights to place mine in the gravel. I grew mine under less than 1wpg, so it should do well with most light.
 
Well I haven't added ferts because I thought the tank was small enough and the lighting isn't very strong although they were in a different tank last week with no fish, so maybe it was that. I'll wait a while to see if it gets worse/better. What nutrients should I be looking into?
 
2 danios, 5 glowlight tetras. right now it is a 14w daylight cfl and a 10w aquarium tubular cfl, but I'm planning to replace the 10w as soon as possible with another 14w
