When planting, do you leave the foam and lead weight on the plant? I planted some plants months ago and left them on. Now the plants have developed roots and I'm thinking of removing the foam and lead weight.

Yes, you should remove the weight and foam.

I've always removed anything that came attached to my plants. Especially metal because if it were to rust or corrode it can leach nasty stuff into the tank.
