It just sounds like you may have. Bacteria tend to colonize in gravel ( substrate) , media pads (filters) all of which you have recently changed or are new

I think you can add plants because your using fish to cycle the tank but I am no plant expert we do have a planted tanks section you can post in it and ask the plant people

Oh I almost forgot do a 25% wc everytime a fish dies because when they do they pollute the water
