Runaway Heater

The 150 watt Visi-Therm Stealth Heater on my 46 gal tank, went crazy yesterday.
The tank temp went from 78'F to 97'F in about 18 hours. I lost 8 cardinal tetras and 4 flame tetras. (sigh) .
The heater was only 8 months old.

Has anyone had a similar problem with this brand/ type of heater?
Also, any recommendations on a replacement unit?
