Do you have a separate tank to move him to? If so I would do that and start treating with either Seachem Paraguard or API general cure until you can better identify what is wrong with him. Are his gills really red? Any spots or injury that you can see?

I agree. Too many fish and not sure they are all compatible? A 20 long is not a lot of tank. You could do 2 schools of 8-10 small community fish like Rummynose Tetra or Cardinal Tetra and a single Dwarf Gourami. I prefer sand myself and just add diy root tabs. All of the plant substrate is made of a porous clay basically and can absorb nutrients which can lead to problems. Like others have sand it's just a matter of personal preference. I like sand and so go my Cory and Loaches.
