Do you have a home water filter of some sort? If not then I'd first test your tap water straight out of the tap at home just to verify that the water in the tank is the same as that coming from your tap. It doesn't matter what numbers the water company gives you if that isn't what is coming...

Something isn't making sense here. :? Your tank has no fish in it and you haven't been adding ammonia. Why do you think your tank was cycled? Your readings are showing ammonia/nitrite/nitrate... have you ever had fish in the tank or have done something else to get ammonia in the tank...
