I would definitely start by doing a good tank change to get the ammonia down to 0. Fish will recover much faster with perfect water conditions. Next, put a double dose of slime coat in the tank to help with recovery. Make sure the water conditions stay perfect and keep an eye on her..

Metronidazole may work, but not effectively at all. It's mainly for treating internal parasites and from my experience it should not be used to combat ich unless it is accompanied by risen temperature.

Ir might be a little extreme but if euthanasia is the other option I would take the fish out and see if you can open it yourself. Maybe treat him with a very light dose of cove oil to make him sleepy and then take him out and take a peek.
