    sounds less like bacteria and more like a parasite. Black Spot or Tang Disease - Black Ich Diagnosis and Treatment try this link and see if the picture matches the black spots on your fish.

    tetra flake foods and some of their pellets I have found they have had worm eggs in them and the more you feed the more worms. They didn't seem to bother the fish but i didn't like them. I had to totally clean out the tank and stop the foods. I had to then clean it again about a month later...

    Sounds like dropsy. This means the fish is not able to process the water it takes in through the kidneys.     Get a good antibiotic that treats cloud eye and get something for parasites and you may try combo of meds if neccessary and as far as a timid fish try covering the tank for a couple of days so he feels confident to eat and swim then try uncovering it more and more and he should be fine.

    it's always safe to assume that the fish is infected. along with good water condtioning treat with broad spectrum antibiotics. take out carbon and raise the temp a few degrees. anytime i see one acting different i immediately add antibiotics and I am a big fan of malachite blue for parasites.
