How long ago did the nem die? If it was recent that could be the culprit. They are poison (literally) to a tank especially a small on when they kick the bucket.

How old is your tank? Was it cycled? Was there any cleaning around (not in) the tank recently using chemicals? Your recent water change, what was the source of the water? Could the change have caused a mini cycle with an ammonia spike? What are your parameters showing?

Kole Tang and two chromis at the moment. It also has a Hammer Coral, Leather Toadstool, Frog Spawn, and a Kenya Tree. I had no cycle as all the live Rock and half the sand came from an established 125 in my home. Any advice or suggestions will be appreciated.

I just went through this with my Sailfin. I would suggest you treat the little fellow a full 28 days before returning him to the display. Research the life cycle of ich and you can see why. After 14 days of treatment they "should" be eradicated but it will take another 14 days to be absolutely sure.
