Can anyone Identify this coral/ anemone. I first noticed it on a coral garden I purchased then found it on a coral branch and now it has moved to a rock. Looks like a polyp when closed up.

Looking into my tank this evening I noticed a worm type creature coming out of the live rock. It is exposed by approx 2" in length and about 2mm in width but it appears to be feeding on algae. Any thoughts on what it is.
