Anyone know what's happening to my plant?

I have 2 fancy goldfish and a bristle nose pleco in my tank but I noticed that one my plants started going white at the top I thought that is just new Young growth but it's been like this for a while I'm starting to think it might be ammonia burn? I do 60%water changes every week the fish appear fine and happy just wondering if this could harm my fish in the long term? ImageUploadedByTapatalk1438202620.921350.jpg

I doubt it is ammonia burn because it would affect the fish first (plants will actually consume ammonia).
It is probably due to inadequate lighting.
Also, when plants are bunched up with the stems crowded together, there is a tendency for the stems to rot (that would start from the bottom up).
